Concerns were raised about a disgraced neurosurgeon more than two decades ago but the warnings were ignored.
Sam Eljamel botched dozens of operations and left patients with lifelong injuries while working in Dundee between 1995 and 2013.
NHS Tayside ignored warnings that he was a “poor doctor”, and it had been believed that concerns about his conduct were first raised in 2011.
However, the General Medical Council (GMC) has now confirmed that it handled a complaint against Eljamel in 2001, 12 years before he was suspended.
